Interscholastic Sports:

Middle School participation in interscholastic sports begins in 7th Grade. The following requirements must be met prior to the first day of tryouts:

The student-athlete must be academically eligible to participate.

Prior to any student participating in practices, inter-school practices, scrimmages, and/or contests, the student-athlete is required to (1) complete a Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE); and (2) have the appropriate person(s) complete the first six sections of the CIPPE Form. Students playing multiple sports throughout the year must complete section seven of the form for each subsequent season no earlier than six weeks prior to the start of the season.

SECTION 1: PERSONAL AND EMERGENCY INFORMATION To be completed by the parent/guardian.

SECTION 2: CERTIFICATION OF PARENT/GUARDIAN To be completed by the parent/guardian.

SECTION 3: UNDERSTANDING OF RISK OF CONCUSSION AND TRAUMATIC BRAIN INJURY To be completed by the student-athlete and parent/guardian.

SECTION 4: UNDERSTANDING OF SUDDEN CARDIAC ARREST SYMPTOMS AND WARNING SIGNS To be completed by the student-athlete and parent/guardian.

SECTION 5: HEALTH HISTORY To be completed by the student-athlete and parent/guardian.

SECTION 6: PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION AND CERTIFICATION OF AUTHORIZED MEDICAL EXAMINER To be completed by an authorized medical examiner (primary care physician, Urgent Care, Minute Clinic, etc.). The Physical may not be authorized earlier than May 1st and shall be effective, regardless of when performed during a school year, until the latter of the next April 30th or the conclusion of the spring sports season.

SECTION 7: RE-CERTIFICATION BY PARENT/GUARDIAN To be completed by the student-athlete and parent/guardian for each subsequent season of competition. If any of the recertification questions are answered “yes”, Section 8 is to be completed by an authorized medical examiner (primary care physician, Urgent Care, Minute Clinic, etc.).

SECTION 8: RE-CERTIFICATION BY LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE To be completed for any student who, subsequent to completion of Sections 1 through 5 of this CIPPE Form required medical treatment from a licensed physician of medicine or osteopathic medicine.

SECTION 9: CIPPE MINIMUM WRESTLING WEIGHT To be completed for wrestlers only by the Neshaminy High School Athletic Trainer at the start of the season.
