Important Reminders

  • All incoming 7th Graders are required to have a TDAP AND Menactra Vaccines PRIOR to entering 7th grade. Please get both immunizations this summer if your student has not already received them. 
  • All 7th grade students are required to have a completed dental examination form on file in the school health office.
  • All 6th grade students are required to have a completed physical examination form on file in the school health office.
  • All medications, prescribed and over-the-counter, require a doctor's order at the beginning of each school year. If your child needs to take any medication while at school, please have your doctor complete the medication order form and return it, along with the medication in its original container, at the beginning of the next school year.

Physical Education Class Exemptions

If your child was injured and has a medical exemption for physical education class or recess, please contact the nurse directly. All excuses must be cleared by the school nurse.

Frequently Asked Questions

  • My child was sent home sick. When can they return?

    • If your child has a fever, they must remain home until fever free for 24 hours without using fever-reducing medication.
    • If your child has been vomiting or had diarrhea, they must remain home for 24 hours once symptoms subside.
    • Please notify your school nurse if your child has an illness that requires them to miss more than 3 consecutive days.
    • Please contact the nurse with any medical concerns, conditions, or need for care while in school. The school nurse will discuss the student's potential needs and gladly plan for your child's care and comfort to ensure academic success while in school.

    My child had an injury. What information do you need?

    • If your child was seen by a healthcare provider for an injury, there may be restrictions or accommodations the nurse should be aware of.
    • Please bring a note from your child's doctor, urgent care or emergency department. The nurse will communicate any special needs to appropriate staff members.
    • If your child may need pain medicine like Tylenol or Ibuprofen during school hours, consider having your healthcare provider complete a medication order form. The nurse is not able to administer over-the-counter medications without an order.  

    My child was prescribed medication and they must take it during school hours. What information do you need?

    • All prescription and over-the-counter medications must have a written order from your child's health care provider in order to be dispensed during school hours.
    • Refer to the medication section of this website for the specific forms you'll need to complete. A doctor's note or letter will be accepted as well.
    • The district physician has provided standing orders to administer Tums, oral pain relief gel, hydrocortisone cream, calamine lotion, anti-itch gel, sting-kill swabs, vaseline, and antibiotic ointment if necessary. Please do not send these in with your child.
    • The nurse's office has liquid and chewable Tylenol (Acetaminophen) and Motrin (Ibuprofen) on hand. These cannot be administered, however, without a written order from your child's healthcare provider. 
    • Contact the nurse with any questions or concerns.  

     

    STATE MANDATED HEALTH SERVICES

      • Physicals – Required for Kindergarten, 6th and 9th grades as well as students new to the district. 
      • Dentals – Required for Kindergarten, 3rd and 7th grades as well as students new to the district.
      • Height and Weight – All grades, annually
      • Vision Screening:
        • Far: all students annually
        • Near: all students annually
        • Convex Lens Test: 1st grade and previously untested
        • Color Vision Test: 1st and 2nd grades and previously untested
        • Stereo Depth Perception Test:  1st and 2nd grades and previously untested  
      • Hearing Screening – K, 1st, 2nd, 3rd, 7th &11th grades 
      • Scoliosis Screening – 6th & 7th grades
      • Body Mass Index [BMI] and BMI Percentile- grades Kindergarten through 12th , annually

      Parents, please note:

      • All screening exams may be rated as pass or fail.   
      • Failure means that the results are outside the expected range but are not a diagnosis of ill health.   
      • Parents will be notified of all failures with regard to health screenings. It is recommended that your child be examined by an appropriate health care provider.  

    Medications in School

    Students attending Maple Point MS follow the same School Board policies as other Neshaminy School District students.

    • No medications, prescription or over-the-counter, may be dispensed unless the school nurse has an order from the physician and written parent permission on file.
    • All medications must be presented to the nurse in a bottle properly labeled by a pharmacist and/or drug manufacturer.
    • Medications must be delivered by the parent.  
    • Students may only carry medication with them if the physician writes the order to include this instruction and a copy is on file in the nurse's office.
